The Mobile Stabilization program provides in-person stabilization for youth in foster and kinship placements with Monroe County foster families. The program uses a two-person team approach, which consists of a Care Coordinator and a Family Peer Advocate. This program works in direct partnership with the Monroe County Department of Human Services (DHS).

About this Program

Monroe County foster youth can be referred to the program by Monroe County DHS After-Hours between 5-11pm daily for immediate concerns that may include conflicts between youth and caregivers, behavioral health crises, or behaviors that pose a threat to themselves or others. The Mobile Stabilization team will screen for stabilization response, which consists of triaging of calls with the foster/kinship family to determine if an in-person response is required. If so, the team will meet with the youth and family at the foster home and begin providing services to help de-escalate the youth and provide a plan for on-going support.

During Assessment and Stabilization, the team uses the Praed Foundation’s Crisis Assessment Tool (CAT) to assess the identified needs of the youth and support decisions made regarding treatment and placement at the time of the crisis. The team will also use a 9-step stabilization procedure that includes elements of assessment, de-escalation process, termination of an encounter procedures, and questions for evaluation. This guidance comes from the US Department of Health and Human Services manual for Crisis Intervention, and is consistent with Hillside’s Therapeutic Crisis Intervention (TCI) and Children and Residential Experiences (CARE) models

The team, along with the youth and family, will also amend or create an Individualized Safety Plan that will assist with avoiding or de-escalating crises in the future. Once stability is achieved, the Mobile Stabilization team will provide the family and caseworker with a list of services and support that can be accessed. There are also opportunities for the team to receive referrals from Monroe County caseworkers for proactive skill building services. These are provided on a short-term basis to help with stability in the foster/kinship home while linkages to other supports are made.

Eligibility

This program is open to youth residing with Monroe County Department of Human Services Foster Families and Kinship Families. Referrals will come directly from Monroe County caseworkers or the Monroe County After-Hours team.
