About this Program

Hillside’s Next Steps: Critical Time Transition Program (CTTP) provides services to support youth who are in need of additional services in order to discharge from local hospitals. Youth must have been boarding for 72 consecutive hours or more within the emergency psychiatric or inpatient psych/medical setting. While enrolled in the program, families will have access to the following services:

  • Care coordination
  • Recreational therapy
  • Family and youth peer advocacy
  • Educational and vocational coordination
  • Mental health counseling
  • Nursing coordination including medication monitoring
  • Behavioral support
  • Supervision and assistance from direct care staff

This program is a hybrid model, offering community-based care management services and access to Transitional Residential Beds. Youth can remain in the program for 6 to 9 months, receiving care management services, with the option to utilize a Transitional Bed for up to 120 days at any time during enrollment in the program.

Next Steps: CTTP is designed for youth who have experienced prolonged stays in the hospital for challenges related to their mental health and/or intellectual disabilities and when other community based programs have not been considered appropriate or the family has declined them. Next Steps is intended to support families who have faced challenges to meet the complex needs of their child, resulting in an extended hospital stay as a means of support and stabilization, due to a lack of other available community resources.

While youth cannot be accepted into the program until they have been boarding for at least 72 hours, referrals can be accepted prior to this timeframe. To be considered, youth must have a long term discharge disposition (ex. return home, group home) and their family/caregiver must be willing to engage in the Next Steps: CTTP program and with community based providers.

Youth served in Next Steps: CTTP have complex needs, including mental health conditions, co-occurring substance abuse and/or intellectual or developmental disabilities. Youth who have been accepted to a higher level of care but are awaiting admission to that program (e.g., RTF) would not be eligible to be served by Next Steps: CTTP. Youth who require 24/7 nursing care due to medical conditions would also not be eligible to be served in this program. The Next Steps: CTTP program is not a locked facility and would not be able to serve youth in our Transitional Beds who present with an elopement risk. Youth served in Next Steps: CTTP must be able to self-administer their medication with supervision and support and be independent with Activities of Daily Living (ADLs). The Transitional Bed program is a restraint-free setting.

This program is open to eligible residents of Genesee, Livingston, Monroe, Orleans, Ontario, Seneca, Wayne, Wyoming, and Yates
