Hillside’s I/DD (Intellectual and Developmental Disability) Family Support Services, located in Rochester, NY, provides intensive, trauma-informed supports for youth with intellectual and/or developmental disabilities who, along with their families, have been unable to access traditional services. The program is designed to stabilize families, prevent child welfare involvement, and improve youth functioning by delivering services in homes, schools, and community settings. This service aims to improve family functioning, increase community supports, and ensure safety and crisis planning.
Participating families will benefit from strength-based, culturally competent, trauma-informed and family-focused supports and services.
About this Program
Program goals include improving daily functioning, strengthening caregiver capacity to support youth with complex needs, and connecting youth and family to long-term services. The program team includes specific roles, all of whom work together with participating youth and families:
- The Behavior Analyst develops intervention plans that reduce challenging behaviors, promote positive coping and assist the youth and family with behavior planning.
- The Skill Builder provides individualized sessions that strengthen emotional regulation, communication, and life skills.
- The Care Coordinator ensures coordinated planning, school and provider collaboration, and connections to long-term resources and services.
- The Family Peer Advocate enhances engagement, reduces barriers, and empowers caregivers through lived experience.
Together, these team members promote improved functioning, reduced risk behaviors, greater caregiver confidence, and long-term stability.
Eligibility
This program is open to youth 12-18 in Monroe County with Intellectual and/or development disabilities (I/DD) and their families.
