For too many youth placed in foster care, the socio-emotional challenges of day-to-day living can make it difficult to plan for a successful future as an independent adult. Hillside’s Monroe County Transitional Services program provides focused supports to help eligible participants make meaningful progress toward their long-term goals.
About this Program
Hillside’s Monroe County Transitional Services program works with Monroe County youth in foster care ages 16-21 whose permanency planning goal is Alternative Planned Living Arrangement. The program focuses on helping individuals obtain the skills necessary to support a successful discharge from foster care, as well as making additional progress towards becoming independent young adults. Participants work directly with a Hillside Care Coordinator who helps them learn and practice skills based on an individual assessment of needs and future goals. Services are provided in both one-on-one and group settings, and include skills development in areas including (but not limited to) the following:
- Finances and Money Management
- Education
- Housing
- Job and Career
- Life Skills
- Parenting
- Transportation
- Community Culture and Social Life
- Self-Care and Physical Health
- Household Management
- Pregnancy and Sexual Health
- Access to Community Resources and Supports
Eligibility
This program is open to youth ages 16 to 21 years, who are in foster care through Monroe County Department of Social Services. Participation in the program requires a referral from Monroe County.
