Hillside’s community-based Peer Navigator services offer eligible Chautauqua County families with an opportunity to enhance their knowledge and advocacy skills, while empowering them to more effectively build upon their strengths and resources and meet their families’ needs.

About this Program

The Peer Navigator program empowers families with a child who is experiencing social, emotional, developmental, medical, substance use, and/or behavioral challenges in their home, school, placement, and/or community to be more effective in meeting their family’s needs. The program aims to enhance the system of care by partnering with youth and families to connect with resources, build natural community supports amongst themselves, educate youth and families and support their advocacy efforts. This voluntary family-driven and youth-guided program is provided by Hillside’s credentialed Family and Youth Peer Advocates.

Eligibility

This program serves families of children and youth ages birth to 21 years.

This program is open to eligible residents of Chautauqua County.
