Hillside’s Therapeutic Foster Care (TFC) services provide a safe and therapeutic family environment for youth who are placed in foster care through the family court system and require a high level of support. The goal of TFC is to reunify youth with their families; however, if the child is free for adoption, then the team works to connect them with a family that wants to adopt. TFC staff also train and certify kinship caregivers as foster parents to provide supports to the youth and relative family and improve permanency outcomes.
About this Program
Hillside’s TFC services include foster family care as well as kinship/relative foster care, care coordination, permanency planning, coordination of health care and behavioral health care, skill building and educational planning.
Specific services include:
- Foster parent recruiting, training, and monitoring by Hillside Home Finders
- Supervision and support from trained and certified foster parents including extensive training on providing trauma-responsive care for the youth in their homes
- Coordination of all services provided to the youth and family with all service providers, family court, and Department of Social Services
- Targeted youth skill development from Hillside skillbuilders
- Permanency planning by trained Hillside Care Coordinators and/or Permanency Specialists
- Educational support and access to community schools by Hillside care coordinators or an education specialist
- Healthcare services are provided by local medical professionals with medical case management and oversight provided by a Hillside Registered Nurse (an RN is on call 24 hours / day)
- Behavioral healthcare is provided by local mental health professionals with coordination and treatment planning provided by a Hillside clinician
- Visitation coaching and supervision of visits as required by court order
- Transportation assistance to appointments and visitation as needed
- Support for all youth and foster families is provided 24 hours a day/7 days a week through a combination of TFC staff and Hillside Service Integration
Eligibility
Youth ages 0 to 21 years
Hillside’s Therapeutic Foster Care services are available to residents of the following counties: Allegany, Broome, Cayuga, Cattaraugus, Chautauqua, Chemung, Chenango, Cortland, Erie, Genesee, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Schuyler, St. Lawrence, Steuben, Tioga, Tompkins and Wyoming.
